Minecraft keep disconnecting.
- Terrain not loading after login
- I keep being disconnected not long after logging enter

I suggest you these:
Use Chunk Builder: Threaded setting in “Video Settings”
Try to decrease the render distance to 10, if I remember correctly it is the Factions world default cap.
Check your internet connection, especially if someone else is using your internet connection to download/upload etc. Restarting the modem is a cliche, but it helps in some cases.
If none of them at least makes the situation better, it could be caused by some mods you are using.
